Nokturia (nykturia)

Nocturia, a common symptom, is defined as waking at night from sleep to void. Although by definition even a single episode of awakening to urinate is nocturia, most clinicians consider nocturia to have significance when a patient voids two or more times nightly. Because nocturia is associated with a variety of clinical syndromes and disorders, the diagnostic approach is often challenging, and treatment may result in only small improvement. Nocturia can be attributed to any disorder or condition that causes low volume bladder void, increased volume of nighttime urinary output, sleep disturbance. Trials of alph-blockers with or without 5-alpha reductase inhibitors in men with nocturia related to benign prostate hypertrophy and trials of bladder muscle relaxants for patients with low volume voids are suggested. A trial of ddAVP for younger patients (65 years of age or less) who, despite other interventions, have bothersome nocturia with evidence of nocturnal polyuria, and who have normal baseline sodium levels should be taken into account.
Nokturia jest to konieczność oddawania moczu w porze nocnej. Występuje powszechnie u osób starszych. Jest jedną z najczęstszych przyczyn zaburzeń snu u ludzi w podeszłym wieku oraz pogorszenia ich jakości życia. Trzy zasadnicze przyczyny prawdziwej nykturii to: oddawanie nadmiernej objętości moczu, zaburzenia czynności pęcherza moczowego, oraz trudności związane ze snem. Pacjenci, u których stwierdza się nokturię, powinni mieć przeprowadzone postępowanie diagnostyczne celem znalezienia jej przyczyny. Dobra diagnoza jest warunkiem możliwie jak najbardziej skutecznego leczenie. Jak dotychczas nie ma ustalonych standardów leczenia nokturii. W leczeniu stosujemy niefarmakologiczne metody polegające głównie na zmianie stylu życia, jeśli jest to możliwe to leczenie choroby będącej przyczyną nykturii, oraz leczenie farmakologiczne (gł. analogi wazopresyny, spazmolityki, antagoniści receptora alfa1 adrenergicznego). Rzadko można zastosować leczenie operacyjne.
